These papers need to be filled out with a Notary present.  We have an "in" with a Notary.  Her name is Vicky Clark, and she works for the Vigo County Health Department in the Vigo County Annex Building across from Fairbanks Park.  Simply park on the north out south side of the building, she is located in the middle.  The office hours are 8-4, Vicky usually takes her lunch from 11-12.Notaries typically charge $2.00, however Vicky is willing to hook us up for free.
